As the team continues to develop, we are looking to recruit a motivated Assessment and review worker to join our assessment and discharge team within the service. The team consists of both qualified and unqualified Social Care, Nursing and Therapy staff. We offer planned and urgent assessments and provide a 2-hour response under Urgent Community Response Criteria as well as supporting hospital and intermediate care discharges.<br><br>The Contact Cares team also provide initial contact, care planning, onward referrals, and signposting services for a wide range of adult clients across health and social care. With a focus that stretches from prevention and early intervention, to reducing hospital admissions and readmissions the work is varied and interesting.<br><br>Newton Community Hospital is a 30 bedded IMC unit, providing Intermediate Care in a community setting, including nursing, medical, therapy and social care interventions for patients who require specialist care and support. Intermediate care is for people whose short-term needs can be addressed within a limited period of weeks, as part of their overall care plan. The aim is to provide assessment and treatment to aid people to recover previous functional ability following illness or injury and promote health, wellbeing and independence supporting them to remain within their own home, prevent admission or readmissions to an acute hospital and prevent discharge to care homes.<br>Onward referrals to other community nursing and therapy teams can be made as part of the discharge and on-going care plan.<br>There is a dedicated multidisciplinary team that support both patients and families including nursing, GP, therapy, social work, and mental health with in-reach support from various disciplines.<br><br>The team operate from 8.00 am until 10.00 pm 7 days per week to include Bank Holidays. Under the Regulations to Mental Health Act 1983, the Council has a statutory duty/responsibility to ensure that AMHP (Approved Mental Health Professional) provision is available over a twenty-four-hour period, 7 days per week.<br><br>During the day, AMHP provision is situated within the Working Age Social Work Team. This provision ends at 17.15 but the current AMHPs are often working past core hours. St Helens Council Emergency Duty Team (EDT) also provide AMHP cover (out of hours) and often Mental Health Act Assessments are passed over to EDT from daytime or, from EDT to daytime services which impacts on both services.<br><br>The successful post holder will bridge the gap where day services end and, EDT begin, providing a continuum of day services from 16.00 to 20.00 hours, five days a week. This post will firm up the statutory responsibilities under the Mental Health Act 1983. It is hoped that the post holder will take the work that on occasions is passed from service to service. This will ensure that we provide a smooth-running service to our service users and NHS colleagues in Mental Health Services, as referrals for Mental Health Act Assessments requested later in the afternoon would be coordinated by the successful post holder, rather than waiting for EDT who arrive on shift at 17.00 hours. You may on occasion be asked to undertake other work that comes under the statutory functions of the Mental Health Act 1983.<br><br>Our current benefits package for this role includes:<br><br>·Additional £4,000 payment paid to those who are AMHP qualified and are added to the AMHP rota. This payment is reviewed every two years. They require support with direct personal care using safer people handling equipment where assessed. The role can often include supporting Service Users to manage their challenging behaviour by following risk assessments and risk management strategies, whilst maintaining and promoting their independence at all times.<br><br>The client group we support require consistent intensive support, due to their level of learning disability and/or physical disabilities/behaviour. Some of our Service Users may experience problems in managing their complex behaviour; therefore, we adopt a multi-agency approach to help promote positive behaviour management, in the safest, ethical, and most appropriate way possible.<br><br>This role will involve working from person centred support plans, risk assessments and the promotion of person-centred planning. Supported Living Workers will also be expected to follow all the policies and procedures that are in place e.g. safeguarding of vulnerable adult’s procedures and attend regular training.<br><br>The role of Supported Living Worker requires total flexibility to support 25 Service Users living in 17 tenancies across the borough of St Helens. Shifts can include sleep in duties, waking nights and a variety of shift patterns e.g. an early shift can start from 7.30am and a late shift working up until 11pm. Some shifts mean a late shift followed by a sleep-in and an early shift the next day, which can equate to 24hrs over those two days. Shift patterns can change to meet the needs of the Service Users. Support Workers need to be flexible as you may need to work in various properties across the service. Shift work includes a proportion of weekend working and Bank holidays which are reflected on rotas.<br><br>The hourly rate for this post is £13.26 to £13.47 per hour<br><br>Applicants must have some experience of working with adults or children with a learning disability, and ideally possess an NVQ level 2 in Health and Social Care or equivalent and be willing to undertake all essential training to meet the Care Quality Commission Standards for the service.
