To work closely with health and social care staff to promote rehabilitation and enablement care post-discharge.</p>\n<p>To work within organisational policy and legislative requirements including the Hospital Discharge Service Policy and Operating Model, Health and Care Act, Human Rights Act, Equality Act, Care Act, Mental Capacity Act and Mental Health Act.  To ensure appropriate information sharing in compliance with GDPR (General Data Protection Regulation).</p>\n<p>To work autonomously within the role, demonstrating expert and effective practice in complex situations managing higher levels of risk. To be able to demonstrate effective multi-disciplinary practice, and work cohesively with partner services and stakeholders, whilst advocating a social care perspective in what can be challenging situations. Staff should have demonstrable experience and knowledge of working either within or with health and social care sectors, understand activities relating to acute and community hospital length of stays and No Criteria to Reside delays and contribute to effective and cohesive system partnership working.</p>\n<p>Staff must be able to offer expertise within the organisation including demonstrating having obtained or be working towards Practice Educator, AMHP or BIA qualifications.
