This service is for those people who may otherwise be at imminent risk of hospital admission.</p>\n<p>The Successful candidate will have a key role to play in supporting the delivery of the Personalisation and inclusion Pathways for Adults and Carers ensuring legal framework of the Care Act 2014. Wirral Council are using the 3 Conversations model (3C¿s) across all Adult Social Care teams. The ethos of the model is to identify individual’s strengths and aspirations to maximise their independence and wellbeing.
