Wirral Council are using the 3 Conversations model (3Cs) across all Adult Social Care teams. The ethos of the model is to identify individual’s strengths and aspirations to maximise their independence and wellbeing.</p>\n<p>You will support and provide professional supervision to qualified Social Workers and unqualified social care professionals in the team, and support with quality assuring assessments with support from Practice Lead colleagues.</p>\n<p>You will be part of a leadership team, providing leadership and management in conjunction with Team Manager and MDT Leads, responsible for some HR tasks, supporting the daily operational needs of the services.</p>\n<p>As an Advanced Practitioner you will be expected to support with complex cases, taking the lead role in ensuring that all operational social work practice involving safeguarding, adult protection and assessment are completed in accordance with relevant legislation, policy and procedures. You will work autonomously and independently to support the team in assessing, formulating, implementing, and agreeing care and support plans with adults and carers.</p>\n<p>You will be responsible for supporting the delivery of health and social care functions for adults/carers for the people of Wirral. You will be responsible for working in partnership with other agencies supporting adults/carers to access services/support in the independent and non-statutory sector. You’ll need to be qualified Experienced Social Worker and registered with Social Work England and have experience working in community social work and an understanding and experience of hospital prevention and discharge pathways.</p>\n<p>Your experience should include Work within Social Work England Standards of registration, professional values and code of conduct, to provide social work input and support to adults/carers/families helping to promote self-determination, choice and autonomy.</p>\n<p>You will have knowledge and understanding of Care Act (2014), Mental Capacity Act (2005) and the Human Rights Act (1998).
