The Role:

We are looking to recruit for a permanent appointment to a Team Manager post within the Hospital Discharge Team.  The successful candidate will be assigned to designated sites across Liverpool.

We are looking for an enthusiastic and committed team manager, who can work agilely, professionally and provide leadership to our social care teams within the hospital.

The Team Manager will lead the hospital discharge team to deliver Adult Social Care statutory duties through strengths based qualitative conversations, our ethos is home first and we are working towards reducing the number of people going into long term care.

We are working towards a set of standards that creates a culture of excellent practice, we are aiming to do this through continuous learning and collaborative development with our colleagues and partners.

As a team manager you will be responsible for overseeing workloads, quality and managing performance, you will enable the team to deliver excellent and safe safeguarding interventions.

You will be integral to the strategic planning of the service, responding to trends to maintain a high standard of service whilst continuously improving the changing environment.

Crucial to this role is the ability to create and sustain positive relationships both internally and externally, including out community social work teams and health partners.
