We are looking for a Senior Occupational Therapist, you must be HCPC registered with a recognised Occupational Therapy qualification.

The Early Intervention and Prevention Team is a fast paced and supportive multi-disciplinary team, working closely with our excellent in-house Intermediate Care Service. We prevent people going into hospital unnecessarily, support them when they come out and provide quality reablement to keep people as independent as they can be. We visit people in their own homes and involve them in all aspects of their support, as well as their families and anyone else important to them. When we are not visiting our clients, we are mostly home based, however we have a fabulous, revamped office space near the centre of Bracknell that you can utilise.
To work with people, including their advocates and carers, to coordinate the identification of their needs. To undertake comprehensive assessments of individuals, including those with diverse or complex presentations, to develop person-centred SMART goals and support plans designed to meet needs in ways that maximises independence, choice and control, and are legal and affordable.
Interviews will be held week beginning 11th November 2024.
