Are you an experienced Social Care Practitioner looking for a new opportunity within a supportive and dynamic team? West Berkshire Council's Adult Social Care Service is seeking a motivated individual to join the Review Team, where you will play a crucial role in delivering high-quality care and support to our local community.
About the Role
As a Social Care Practitioner within the Review Team, you will be responsible for:
- Working collaboratively with individuals to assess their social care needs and provide appropriate reviews and interventions.
- Conducting reviews of care packages to ensure that individuals are receiving the necessary support to achieve their desired outcomes.
- Building positive relationships with service users, carers, and external providers to promote effective communication and information sharing.
- Advising and supporting individuals in accessing relevant services and resources.
- Working alongside other professionals to ensure holistic care and support is provided.
- Contributing to the continuous improvement of the Review Team by participating in team meetings and sharing best practices.
About You
The ideal candidate will possess the following skills and attributes:
- A solid background in social care, with experience in delivering assessments and reviews.
- Knowledge of relevant legislation, regulations, and national standards.
- Excellent communication and interpersonal skills, with the ability to build rapport with service users and stakeholders.
- Strong organisational and time management abilities, with the capacity to manage a diverse caseload.
- Problem-solving mindset, with the ability to assess risks and make informed decisions.
- Ability to work both independently and as part of a team, demonstrating collaboration and mutual support.
