This role was formerly known as early help worker.</p>\n<p>The purpose of the Adolescent Multi-Disciplinary team is to provide adolescents aged 11-17 with specialist intervention for a period of up to 6 months. You will play a pivotal role in supporting young people at the earliest opportunity with the aim of preventing statutory intervention.</p>\n<p>We work with young people who may be experiencing an array of difficulties, examples include criminal or sexual exploitation, mental health difficulties, low level criminality, family conflict, missing from home and substance misuse to name a few.  </p>\n<p>You will be based in the Adolescent Multi-Disciplinary team which comprises of a Team Leader, 4 Practitioners, 2 Youth Mentors and 1 Senior Youth Mentor. Job Introduction

An exciting opportunity has arisen for an MDT Practitioner to join our Adolescent Multi-Disciplinary Team. This role was formerly known as early help worker.
