Job description
Are you passionate about providing exceptional care and support to individuals with dementia? Do you thrive in a collaborative and supportive working environment? West Berkshire Council is seeking a dedicated Night Dementia Practitioner to join our Adult Social Care Service at Birchwood Care Home.
The Role
As a Night Dementia Practitioner, you will play a vital role in delivering high-quality, person-centred care to our residents during the night hours. You will work closely with the Registered Manager and other members of the care team to ensure that the needs of individuals with dementia are met, providing support and guidance to enhance their well-being and quality of life.
Key responsibilities include:
- Ensuring the delivery of compassionate and dignified care to residents
- Monitoring and assessing the changing needs of individuals, providing appropriate interventions
- Collaborating with the care team to implement personalised care plans
- Providing support to residents during the night, addressing their physical and emotional needs
- Maintaining accurate and timely documentation of care provided
- Working in partnership with families.
The ideal candidate will possess the following skills and attributes:
- A genuine passion for working with individuals with dementia and/or nursing care needs
- Excellent communication and interpersonal skills, with the ability to build positive relationships with residents, families, and colleagues
- Strong observational and assessment skills, with the ability to identify changes in individuals' needs
- Ability to work effectively as part of a team, as well as independently
- Knowledge and understanding of dementia care best practices
- Relevant qualifications and training in health and social care
This role is fixed term until February 2026
