The Role:
An exciting opportunity has arisen to join the Home First/Outreach service within Liverpool City Council. The service is based at Crown Hub Resource Centre. The reablement team provide short term support to adults over the age of 18 who have been identified as having reablement goals. The Reablement Assistants role adopts a strength-based approach, supporting and encouraging a person’s independence. This may involve a therapy plan, providing direct care and support, or signposting to other services. The team deliver care and support throughout the borough.
The role of Reablement Assistant in the community is to work with individuals to regain daily life skills to enable them to live independently in their own homes.
- As a Reablement Assistant you will assist individuals to retain their independence through person centred planning to ensure a full range of co-ordinated services are delivered
- You will liaise and work alongside other professionals to minimise risk and enable individuals to remain in their own homes
- You will carry out administration of medication as and when required
- You will be part of a team of staff working flexibly over 7 days, between 8am and 10pm on a rota of shift patterns
