The Role
We are seeking a dedicated and experienced Occupational Therapist to join our TEC & Reablement Intervention (TRI) Team. The TRI team are responsible for providing a therapist led reablement service that supports people as part of the Adult Social Care department.
In this role, you will complete assessments and work collaboratively with people who use our service as well as others in the multi-disciplinary team. Identifying meaningful goals and outcomes as well as equipment, therapy interventions and reablement visits to deliver a short term (usually up to 6 weeks) service. The team work predominantly out in the community, within people's homes with some interventions taking place in care homes or within a hospital environment.
Key Responsibilities:
This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.
Valid driving licence and own transport, or be able to provide alternative, suitable method of travel is an essential condition for this role.

The team work predominantly out in the community, within people's homes with some interventions taking place in care homes or within a hospital environment.
Key Responsibilities:
- Independently manage a caseload of people receiving a reablement service, a technology enabled care (TEC) and/or therapy short-term intervention
- Participate in a duty rota and triage referrals into the team
- Work as part of a wider MDT including domiciliary care providers, care homes, and community health services
- Support hospital discharges and participate in a rota working within the Transfer of Care Hubs (TOCH) for UHBW and NBT hospitals.
- Undertake specialist OT assessments including, but not limited to, functional assessment, moving and handling, falls and risk assessments
- Undertake own CPD, learning, development and sharing best practice; participating with in-service training across the team
- Participate in formal and informal supervision of students and practitioner staff

Essential Criteria:
- Qualified Occupational Therapist with HCPC
- Experience of undertaking functional assessment and risk assessment related to activities of daily living (ADL's)
- Experience of setting up person centred rehabilitation and/or care plans
- Experience of using SMART goals to deliver interventions within a health or social care setting
- Ability to carry out mental capacity assessments and best interest decisions
- Ability to apply a risk -enablement approach focussed on people's choice and wellbeing when recommending interventions
- An innovative and creative approach to Occupational Therapy practice with strong problem solving-skills
- Knowledge of Technology Enabled Care (TEC) including telecare, telehealth and high street technology devices
- Ability to work as a lone practitioner within a community setting
Desirable Criteria:
- Experience of working in a reablement, intermediate care or community rehabilitation setting
- Experience of working in social services setting
- Experience of discharge planning including home/access visits
- Experience of implementing Technology Enabled Care
