About the team
Brokerage Solutions Team is committed to sourcing services for people supported by Hertfordshire Health and Social Care colleagues. We are looking for a hardworking, resilient, passionate, and experienced person to apply for the post of Home Finder and join our dynamic team.
You will be part of a team responsible for how Brokerage Solutions Team performs, meeting standards set by the needs of Adult Care Services for an efficient team that delivers in a timely and professional manner.
Brokerage Solutions Team provides support to Social work professionals who are working with individuals who have been assessed as meeting criteria to access social care support for community and residential services. As a Homefinder you will predominately be dealing with long stay placement requests but also will be involved in Prevention of Hospital Admissions and urgent respite requests. This involves having strong communication skills as you will be working not only with internal stakeholders but external stakeholders such as Functional Mental Health Teams, Hospitals, CHC and Care Homes.
You will be committed to delivering a high-quality service in a demanding role.
You will need to be fully up to date with all IT skills required or will have the proven ability to learn them very quickly. You will need to be able to prioritise cases and work under pressure.

About the role
- Managing and prioritising caseload.
- Commissioning services accurately
- Sourcing appropriate services and liaising with Care Managers as well as direct contact with clients and families.
- Working collaboratively with stakeholders to meet request in timely manner and within the budget set.
About you
Essential:
- Experience of working in Social Care and understanding of the Care Act.
- Strong communication skills with the confidence to speak directly to clients and family members, both verbally and written with the ability to establish strong relationships with providers.
- Must be self-motivated and able to work in a fast-paced, pressured environment.
- Working as part of a team and individually
- Excellent organizational I.T and time-management skills.
- Knowledge of Microsoft applications and liquid logic systems (ACSIS)
- Outstanding organisational and prioritisation skills
- Knowledge of Brokerage
- Knowledge of ACSIS.
