The Adult social work/Occupational Therapy teams are responsible for the Short- & Long-Term delivery of person centred support and an asset-based care planning/ equipment provision for Adults over the age of 18 years.</p>\n<p>Halton Borough council’s expectations are high, and you will need a strong commitment to providing first-class person-centred care to Halton residents and support to their carers.
