Through care planning we strive for increased independence, choice and control by enable people to direct their own support.<br><br>You will gain extensive experience in the field of Adult Social Care, Safeguarding practice and Multi-Disciplinary working.<br><br>You may be required to visit service users in the community to undertake reviews, so a driving licence would be essential as is the ability to respond to situations quickly and flexibly.<br><br><strong>We offer a 7 day service and currently operate a 1-weekend-in-8 rota across the Hospital Team </strong>to improve outcomes for service users. You'll be part of a service-wide rota to provide cover on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after covering weekends.
