The Post
The role will be to lead projects and contribute to the delivery of programmes across adult social care and health working collaboratively with our Community Service Provider and other Integrated Care Partners in Brent.
The post holder will be expected to work closely with local care home providers and key stakeholders to develop and align with the ambitions of the Enhanced Heath in Care Homes and social care to support people living in care homes.
In addition, the post holder will be supporting the Integration and Delivery Team with managing a variety of complex and exciting community healthcare contracts to develop and improve healthcare services. The role will support hospital discharges functions from acute hospitals and interim Care Home placements.
The Person
We are looking for a proactive project manager with a proven track record of delivering improvements of health care services and a can-do attitude.
You will be required to build strong, lasting relationships with staff in the NHS and Council, while navigating through the complex landscape of the health and social care system.
