As a Senior Early Help Worker within the Children’s Help and Advice Team, you will play a pivotal role in managing the transition of cases between Children in Need (CIN), Child Protection (CP), and Early Help services. Your primary focus will be to ensure that Early Help Plans are promptly initiated, followed up, and supported by relevant services. This role is integral in ensuring that families receive timely, coordinated support to prevent issues from escalating.
Main Duties:
- Case Transfer Management: Facilitate smooth transitions between CIN, CP, and Early Help services, ensuring continuity of care for families.
- Early Help Plans: Initiate and monitor Early Help Plans, ensuring active involvement of families and professionals.
- Identifying Lead Professionals: Work with multi-agency teams to identify and support Lead Professionals for each family.
- Service Coordination and Signposting: Maintain a thorough understanding of available services and ensure families are referred to appropriate support.
- Step Across Weekly Meetings: Lead and coordinate weekly meetings to review transitioning cases.
- Partnership Working: Build and maintain strong relationships with internal and external partners.
- Safeguarding and Risk Management: Identify and escalate safeguarding concerns, ensuring timely interventions.
- Monitoring and Reporting: Maintain accurate case records and contribute to data analysis to monitor outcomes.
