About the team
We are looking for Senior Social Workers across the following teams:
Adult Disability Team
Please note within Hospital Teams, we offer a 7 day a week service and currently operate a one weekend in 8 rota across the hospital team
About the role
As a Senior Social Worker you will be responsible for overseeing and supporting the delivery of social services to communities or specific areas.
Responsibilities:
About you
You will be a SWE registered Social Worker, at least 2 years post qualified, who has successfully completed an ASYE program working to the Social Work England professional standards. You will undertake The Care Act assessments for eligible current and historical cases, using our Connected Lives strategy to review and manage packages of care.
You will also actively undertake a specialist role, e.g. AMHP, Best Interest Assessor or Practice Educator, have a place secured on a course, or commit to undertaking this as soon as appointed.
The role of a Senior Social Worker is crucial in the discharge to assess model, which aims to improve outcomes for people by reducing the length of hospital stays and promoting independence at home. - Managing schedules and handling issues or concerns within the community
- Providing emotional support and guidance to individuals and families

- Making referrals to other agencies and participating in multidisciplinary teams
- Maintaining accurate records and preparing reports for legal action
