About the team
The Transforming Care Team aim to enable autistic people and people with learning disabilities to live ordinary lives in the community.
You will be working in a dedicated team working with people on the Dynamic Support Register (DSR) and with the Community (Education) Treatment Review programme (C(e)TR). Together we work to help prevent admissions to mental health hospital and work with clinical and community teams to plan sustainable discharges.
About the role
Whilst the role is for an adult worker, part of the role will be assessing and planning for those who are 16 years old plus, this is to ensure personalised and well-planned transitions to adult services take place in a timely way.
You will work at the forefront of best practice to deliver personalised support to vulnerable young people and adults with the most complex care needs. Essential to the role is the ability to see the potential in people and take a proactive approach to risk management and problem solving despite being challenged to the contrary.

About you
It is essential that to have the following:
- Qualified and Social Work England Registered
- At least 1 years post ASYE experience
- Strong written and verbal communication skills
- Knowledge of legislation, guidance, policy, and procedures
- Proficient in the use of Microsoft IT package
- Full UK Driving Licence and regular access to a vehicle
- Good working knowledge of Autism, Transforming Care and personalisation
- Experience and understanding of BASW capability framework
- Strong assessment skills and Care Act knowledge
