They support patients who are ready for discharge into the community from the South London and Maudsley NHS Trust.  </p>\n<p>You will play a crucial role in supporting individuals to achieve recovery, reablement, and rehabilitation within a trauma-informed and psychologically informed environment (PIE). Working closely with residents/participants, you will create personalised support plans, offer practical assistance, and deliver tailored interventions to help them achieve their goals.</p>\n<p>Your efforts will focus on empowering residents/participants to reach their desired outcomes, ensuring they have the necessary tools and resources for positive progress.
