Within Sefton we have dedicated Multi-Disciplinary Teams which have been newly developed to support individuals who have complex needs and can support them out of being stuck in a negative cycle.</p>\n<p>The position will be aligned to our Adult Social Care First Contact Team, but link in with homeless services and the Complex Lives MDTs.
