Our Team:

We are looking to recruit a Community Care Worker, to work within our Adult Review Team based in Municipal Buildings, Widnes. This is a fixed term position for 18 months.

The Adult Review Team is an Adult Social work team responsible for the Long-Term delivery of person centred support and an asset/strengths based support planning for Adults over the age of 18 years. The focus of the role will be supporting Adult Social Care with the completion of social care reviews and assessments.

Halton Borough council’s expectations are high and you will need a strong commitment to providing first class person centred care to Halton residents over 18 and support to their carers.

You will be passionate about working with a range of service user groups (older people, adults with learning disabilities and adults with physical and sensory difficulties, challenging discrimination whilst promoting independence.

You will have a recognised Social Care Experience, within a Care Management team or a social Care setting,  good interpersonal and communication skills, a flexible approach and a willingness to continually improve your practice; in addition, you must hold a full driving licence.
