Making A Difference as An Advanced Practitioner
An opportunity has arisen for a driven and innovative qualified Assistant Team Manager / Advanced practitioner/ Social Worker to join Integrated Health and Social Care within Contact Cares, Hospital Discharge Service. This large and busy team that continues to develop employing forward thinking innovative team members to include senior social workers, and unqualified health and social care staff with focus on early intervention to improve lives, promote independence and facilitate timely discharge from hospital.
The team carries out assessments, care planning, for a wide range of adult patients working closely with staff from across health and social care.
