As the team continues to develop, we are looking to recruit a motivated Assessment and review worker to join our assessment and discharge team within the service. The team consists of both qualified and unqualified Social Care, Nursing and Therapy staff. We offer planned and urgent assessments and provide a 2-hour response under Urgent Community Response Criteria as well as supporting hospital and intermediate care discharges. </span></span></span> </span></span></span></p>\n<p class=\"paragraph\"><span><span><span><span><span><span>The Contact Cares team also provide initial contact, care planning, onward referrals, and signposting services for a wide range of adult clients across health and social care. With a focus that stretches from prevention and early intervention, to reducing hospital admissions and readmissions the work is varied and interesting. </span></span></span> </span></span></span><br><br></p>\n<p><span><span><span><span><span><span>Newton Community Hospital is a 30 bedded IMC unit, providing Intermediate Care in a community setting, including nursing, medical, therapy and social care interventions for patients who require specialist care and support. Intermediate care is for people whose short-term needs can be addressed within a limited period of weeks, as part of their overall care plan. The aim is to provide assessment and treatment to aid people to recover previous functional ability following illness or injury and promote health, wellbeing and independence supporting them to remain within their own home, prevent admission or readmissions to an acute hospital and prevent discharge to care homes.</span></span></span></span></span></span></p>\n<p><span><span><span><span><span><span>Onward referrals to other community nursing and therapy teams can be made as part of the discharge and on-going care plan.</span></span></span></span></span></span></p>\n<p><span><span><span><span><span><span>There is a dedicated multidisciplinary team that support both patients and families including nursing, GP, therapy, social work, and mental health with in-reach support from various disciplines.</span></span></span></span></span></span></p>\n<p class=\"paragraph\"><br><span><span><span><span><span>The team operate from 8.00 am until 10.00 pm 7 days per week to include Bank Holidays.
