The role

About the Role
As a Deputy Team Manager in the Transfer of Care Team, you'll provide vital leadership and supervision to a dynamic, fast-paced hospital discharge team based at King's College Hospital. Reporting directly to the Team Manager, you'll play a key role in:
- Leading a team of Social Workers and other professionals to deliver high-quality, person-centred services.
- Managing safe and timely hospital discharges, working closely with Health, Housing, and voluntary sector partners.
- Overseeing safeguarding decision-making, including triaging referrals and implementing protection plans.
- Supporting mental capacity assessments, DoLS, and best interest decision-making.
- Ensuring effective service delivery by managing workloads, allocating cases, and monitoring performance.
- Providing expert guidance, mentoring, and development opportunities to support your team's growth and maintain high standards.

About You
To excel in this role, you'll be a qualified and registered Social Worker with:
- Proven experience in managing teams within Health and/or Social Care environments.
- Strong leadership skills, with the ability to guide decision-making and inspire your team.
- A robust understanding of safeguarding procedures, Mental Capacity Act, and DoLS.
- Exceptional communication and relationship-building skills to collaborate with partners across sectors.
- A commitment to strengths-based approaches, promoting independence and positive risk-taking.
You'll demonstrate a passion for achieving the best outcomes for Southwark residents, empowering individuals to regain their independence and live fulfilling lives.
