The Integrated Hospital Discharge Service's purpose is to support people transferring from hospital to recover and maintain their independence and wherever possible, to return and stay in their own homes. You will lead on Hospital Discharge Practice, inspiring, developing, nurturing, and supporting the way we work with people. You will lead and manage a multi-disciplinary staff group of managers and social care professionals, to achieve positive outcomes and fulfilled lives for people leaving hospital and whilst recovering post discharge. You will work with teams to enable people to achieve these outcomes working across health and social care organisations seven days a week.
As Deputy Head of Service, you will contribute to the strategic direction of the Integrated Discharge Service as well as the development of practice, procedures, and policy, both internally to Hertfordshire County Council and with system partners as we continuously develop our system approach to Hospital Discharge

About you
Essential
- Qualification - Qualified Social Worker, Occupational Therapist or Nurse.
- Professional registration - SWE/HCPC or NMC
- Understanding of social care legislation
- Management/leadership experience
- Experience in hospital work at a management / leadership level
- Ability to support 7 day per week service delivery
- Team or Service Manager experience in hospital discharge
- Car driver and access to a car (unless disability prevents this)
