Are you passionate about improving the lives of individuals with dementia? Do you have the skills and expertise to provide high-quality care and support to those living with this condition? If so, West Berkshire Council has an exciting opportunity for you to join our Adult Social Care Team as a Dementia Practitioner.
As a Dementia Practitioner, you will play a vital role in delivering specialist dementia care within our Birchwood Care Home. Reporting to the Deputy Manager, you will be responsible for:
- Providing expert advice and guidance on dementia care to both residents and colleagues
- Conducting assessments of individual needs and developing-person-centred care plans
- Ensuring the highest standards of care are maintained, promoting dignity and independence
- Supporting residents and their families through the challenges associated with dementia
- Implementing evidence-based interventions to enhance the well-being of individuals
- Collaborating with external professionals to ensure comprehensive care delivery
The ideal candidate will possess the following skills and attributes:
- A thorough understanding of dementia and its impact on individuals
- Experience in providing direct care and support to people with dementia
- Excellent communication and interpersonal skills, with the ability to build positive relationships
- Strong assessment and care planning abilities
- The capacity to work collaboratively within a multidisciplinary team
- Knowledge of relevant legislation and best practices in dementia care
- A compassionate and empathetic approach, demonstrating respect for individuals' rights
