We are seeking a dedicated and experienced Social Worker (Continuing Health Care) to join our Team. The successful candidate will be responsible for providing high quality social work support to adults with complex needs, including those eligible for Continuing Healthcare. You will be based alongside our Hospital Discharge Team in Whiston Hospital but expected to conduct visits borough wide and occasionally out of borough.
