This may include financial, benefits advice, signposting and helping them be recognised, respected, and heard as carers. <br><br><b>About the role</b><br><br> You can make a significant difference to the lives of our patients and their carers by providing support, advice, guidance and respite incorporating risk reduction, safeguarding and positive risk taking. You will provide a carers assessment using;<ul>\n<li> Analytical skills to inform assessments, decision-making and intervention </li>\n<li> Assessing and supporting carers, promoting creative solutions including direct payments and contingency planning </li>\n<li> Support planning to ensure that outcomes identified at assessment are met through the provision of support, equipment, minor adaptations and enabling programmes </li>\n<li> Actively engaging with carers as individuals and through organisations and community groups. Where necessary undertake carers assessments and provide relevant information, advice and appropriate support </li>\n<li> Working in partnership and developing relationships with local community groups, organisations and community members (in all settings) to achieve positive outcomes for Hertfordshire carers and citizens </li>\n<li> Professional skills to inform assessment, decision making and interventions in line with professional expectation and supports others to do the same </li>\n</ul><strong>You will</strong><ul>\n<li> Ensure effective safeguarding and risk management </li>\n<li> Addresses adversity and social exclusion </li>\n<li> Promote independence and autonomy </li>\n<li> Look to prevention and early intervention where appropriate </li>\n<li> Demonstrate HCC Values and Behaviours and Professional Standards </li>\n<li> Have a Professional understanding of the care act within your general responsibilities </li>\n</ul>You will ensure effective safeguarding and risk management, when an adult is at risk of social exclusion. Assisting people to deal with adverse circumstances such as: poor health, poverty, inadequate living conditions; as well as maximising the strength of individuals, their families and their communities. <br><br> You will actively promote Equality and Diversity and challenge discrimination. <br><br> Carer Practitioners will endorse and act in accordance with the principles of patient personalisation, ensuring that care and support are person-centred and as far as possible put the people with whom they work in control of their lives.
