We have opportunities available for Level 2 Community Care Officers in the Spalding area team on a full time basis and an 18.5 hour - fixed term position (for 3 years) in the South Review Team, work base to be negotiated.
About The Role
You will have a varied and interesting caseload, which will require partnership working with other agencies. This will involve assessing and managing identified risks and considering any informal support available.
You will be required to assess for eligibility in accordance with the Care Act 2014 and where needed, arrange cost-effective packages of support and work in a strength based, person centred way. Creative thinking will be required to determine whether support such as voluntary services or telecare/tech can be used to meet support needs.
The work bases are Spalding and Stamford; travel to appointments to meet the needs of the service is required, and mileage claims are paid to and from appointments.
