To develop and manage a defined caseload by means of identification, referrals, assessment and evaluation of health needs and implement Public Health strategies in partnership with multi agencies within the localities.</span></span></span></span></p> <p><span><span><span><span>​The successful candidate will have the leadership skills required to manage a team of specialist community practitioners, registered nurses, and organise team workload.
