We are looking for <strong>Care and Support Workers</strong> to join <strong>The Reablement Service</strong>, a multi-disciplinary team within our care home division.</p>\n<p>Reporting to the Registered Manager you will provide care and support to adults age 18+ within their own homes across Halton.  Under the supervision of Care and Support Coordinators you will follow individual strength-based care plans and risk assessments, administer medication, promote independence, monitor health and well-being and complete written records to support progress and reviewing.</p>\n<p>Our Care and Support Workers provide short-term periods of rehabilitation, supporting adults to retain or regain their independence, or provide evidence to support assessments of longer-term needs. Working closely with therapists CCWs and social workers.</p>\n<p>You will be home based and work across Halton, <strong><em>this requires the use of your own vehicle</em></strong>.</p>\n<p>We are looking for caring, hardworking, compassionate, and innovative individuals who can ensure that care and support is provided to a high standard and in accordance with care and support plans.  Working as part of a multi-disciplinary team, successful applicants will need to be confident in delivering a strengths-based approach to their work.  Applicants must be easily adaptable to the ever-changing needs and demands of the service.</p>\n<p>Care and Support Workers work on a two-week rolling rota, which includes both early and late shifts between the hours of 07:30 and 23:00, including every other weekend.  There are several contracts available from casual staff, to a range of part time hours and full-time hours.</p>\n<p>Candidates require experience and knowledge of working within the care sector, preferably holding an NVQ level 2 or equivalent in a care related discipline.  Candidates must have good written and spoken English, possess excellent interpersonal skills and can effectively communicate with and listen to others.
